This Program Enrollment Agreement ("Enrollment Agreement") is between Rep Lab LLC ("Rep Lab") and the parent or legal guardian ("Guardian") accepting enrollment on behalf of the minor athlete ("Participant"). It identifies the specific services purchased and incorporates the Rep Lab Participant Service Agreement, Assumption of Risk & Release ("Master Agreement"), in the version affirmatively accepted with this enrollment.

2. One-Time Enrollment; No Automatic Renewal
This purchase covers only the program cycle and sessions identified in Section 1. It is not a subscription and does not renew automatically. Participation in a later cycle requires a separate registration, acceptance of that cycle’s Enrollment Agreement and applicable Master Agreement terms, and payment. Updates to future prices or schedules do not change this accepted enrollment.
3. Payment, Changes, and Withdrawal
Guardian agrees to the price and payment terms stated above. Program withdrawal, missed sessions, credits, refunds, program cancellations, substitutions, and reasonable schedule changes are governed by Sections 4–6 of the accepted Master Agreement, subject to applicable law. Any program-specific exception must be clearly disclosed above and affirmatively accepted.
4. Incorporation of Master Agreement
Guardian acknowledges receipt of and separately agrees to the Rep Lab Participant Service Agreement, Assumption of Risk & Release, including conduct and safety requirements, health and medical clearance procedures, assumption of risk, the adult-only release of claims, communications, and the separate optional media-consent choice. The Master Agreement and this Enrollment Agreement are read together. The program-specific details in Section 1 control those details for this purchase; the Master Agreement otherwise applies unless a lawful exception is expressly disclosed and accepted.
5. Guardian Acceptance and Electronic Record
By signing or electronically affirming acceptance, Guardian confirms authority to enroll the Participant, reviews the exact program details stated above, authorizes participation for this cycle, and agrees to the one-time enrollment and payment terms. The Master Agreement is presented for separate affirmative acceptance during registration.
